Community Dietetics

The Community Dietetic service offers management and support to domiciliary patients with a GP in North West Surrey (NWS) requiring enteral feeding in community locations and those meeting referral criteria who are unable to attend outpatient clinics.

Access the service

Community patients who think they need to see a dietitian can be referred by their doctor, community nurse, practice nurse, health visitor, district nurse, speech and language therapist, physiotherapist or occupational therapist. 

Referrals need to be emailed to csh.SPAreferrals@nhs.net

  • social services
  • consultants
  • other healthcare professionals

In addition, people with long term conditions can self-refer.

A service is provided to inpatients at Walton and Woking community hospitals. The Dietitians work closely with SLT colleagues, palliative care teams, local hospices and other community healthcare professionals to ensure that patients receive appropriate nutrition and hydration, provided in a safe and supported environment. Patients are regularly reviewed and assessed until agreed outcomes are reached. The team also works closely with the NWS Care Home Support team to provide training and support to care homes across NWS.
