Rapid Response Team
If you require an urgent assessment at home, your GP or other health or social care professional can refer you to our Urgent Community Response service.
The team comprises nurses, occupational therapists and physiotherapists and will respond within two hours of a referral being made to enable you to recover from an acute episode of illness, thus avoiding the need for a hospital or A&E admission.
The clinician will provide a holistic assessment of your needs, arranging equipment and support as required, and will liaise with other services where necessary to ensure you are maintained safely in your home environment wherever possible.

In a survey of 120 patients using the Rapid Response Service, 98% said they felt they were treated with dignity.

- Urinary tract infections (UTIs)
- Exacerbation of long-term conditions such as Chronic Obstructive Pulmonary Disorder (COPD)
- Cellulitis
- Chest infections
- Falls (no injury or with minor injury)
- Sudden reduction in mobility
- Failure to cope
- End of life care.
