These conditions include:

  • MND – including Amyotrophic Lateral Sclerosis (ALS), Bulbar onset MND or progressive bulbar palsy (PBP), Progressive muscular atrophy (PMA), and Primary lateral sclerosis (PLS).
  • Huntington’s Disease.

Who is eligible for the specialist complex neurological practitioner service?

The service is for anyone:

  • over the age of 18
  • with a confirmed diagnosis of one of the above conditions
  • who is registered with a GP in the North-West Surrey area.

How do I get to see the specialist complex neurological practitioner?

With consent, referrals can be made by a neurologist, GP, or other healthcare professional.
