About our service

We help you recover safely at home after your hospital stay.

Our aim is to support you to regain independence and reach your recovery goals.

Our team includes nurses, occupational therapists, physiotherapists and rehabilitation assistants. We work 7 days a week.

How the service works

  • You have been referred to our team after leaving hospital
  • We will talk with you about your needs
  • We will arrange an assessment if needed

This service is for adults (18+) registered with a North West Surrey GP.

What to expect

Before you leave hospital

A therapist will assess you and talk with you (and your family if needed) about your short-term recovery goals.

After you get home

📞 Phone call

  • We aim to call you within 24 hours of you arriving home
  • We will talk about your recovery goals and whether you need support from us
  • Some people may not need a visit if they are already managing well

🏠 Home visit (if needed)

  • If you need support, we aim to visit within 7 days
  • We will agree a plan with you to help you meet your goals

We may also suggest or refer you to other services if helpful.
