Thames Medical Hub - Patient Information

The Thames Medical Hub, based at Walton Community Hospital, provides proactive coordinated care for older people with frailty and multiple long-term conditions who are registered with GPs in the boroughs of Runnymede and West Elmbridge.

The Hub service is provided by a multi-disciplinary team of healthcare professionals.

Who is it for?

  • Medically stable patients with complex health and care needs
  • People who may benefit from both health and social care support
  • Those at risk of health or social care crises

Who will I see?

The Thames Medical Hub team consists of wellbeing coordinators, matrons, GPs, a pharmacist, a mental health practitioner and administrative staff. 

People on the caseload can be seen in the Hub or at home where necessary.
