About this service

This service provides support, advice and education to people with type 1 and type 2 diabetes.

This service offers support to family members, care givers and healthcare professionals as well.

Who is it for?

The The Community Diabetes Specialist Nurses provide specialist care for adults with diabetes in the community, in North West Surrey.

Who will I see?

Patients referred to the service will be reviewed by a Diabetes Specialist Nurse. The nurses are:

  • Susan Cable - Lead Diabetes Specialist Nurse
  • Rosie Marks - Senior Diabetes Specialist Nurse
  • Oluwabukola Martins-Akande (Buky) - Senior Diabetes Specialist Nurse

Where is this service provided?

Consultations can be provided face-to-face or virtually, depending upon a person’s individual needs, preferences and requirements.
