This service provides specialist nursing for people with chronic heart failure.
Heart failure nurse specialists provide the management of chronic heart failure, including medication optimisation and symptom management based on best evidence and guidelines. It includes education around self-management and recognising early signs or symptoms of heart failure or when to seek additional support or advice. We aim to promote a good quality of life and avoid hospital admissions.
This service is for all adults with a confirmed diagnosis of heart failure through a recent echocardiogram.
Heart failure nurse specialists and a health care assistant will carry out assessments, and review your condition. We work closely with GPs, cardiologists and other community services.
We offer clinic appointments and visit patients, who are housebound, at their home.
