About this service

Intervention is based around patient-centred goals to maximise independence in daily life.

Referrals can be made to other services to facilitate this process.

Who is it for?

Our Community Rehabilitation Teams provide rehabilitation for people with neurological conditions, elderly and orthopaedic.

Who will I see?

The initial assessment is completed by a qualified therapist and an appropriate treatment plan is then agreed.

Intervention may involve advice, exercises, and or be functional activity focused. Intervention can range from 1 to 6 sessions, depending on the assessment outcome and patient goals.

A self-management approach is promoted.

Where is this service provided?

The team's Physiotherapy and Occupational Therapy assessments are completed in the home environment or in an outpatient setting.
