About this service

We support individuals with a wide range of needs, including those with long-term conditions, complex nutritional requirements, or those receiving palliative care.

Who is it for?

The community dietetic service offers dietetic management and support to domiciliary patients registered with a GP in North West Surrey (NWS).

This includes:

  • Patients receiving enteral tube feeding at home, in care homes or other community settings
  • Some enterally fed patients who are able to travel, may be seen in a specialist clinic at St Peter’s Hospital
  • All patients who meet referral criteria but are unable to attend outpatient clinics.

Who will I see?

Our team of registered dietitians work closely with:

  • Speech and Language Therapy (SLT) colleagues
  • Palliative care teams
  • Local hospices
  • Community nurses and healthcare professionals
  • The North West Surrey Care Home Support Team to provide training and support to care homes across NWS

Where is this service provided?

  • In patients’ homes or care homes across NWS
  • At Alexandra Ward, Woking Community Hospital, for inpatients
  • At St Peter’s hospital for selected enteral feeding patients
